Please download the order form (using Adobe Reader), fill it in as completely as possible and include it with your quilt. You may drop-off your quilt (by appointment only) or mail it in.

• Please e-mail me for scheduling before you send your quilt!
• Once I receive your quilt, please allow 6-8 weeks for your quilt to be completed.
• Payment for Services can be sent along with your quilting order or I can send you an invoice upon completion. Payment must be received before I will mail out your quilt.
• Pack your quilt in a sturdy box large enough for the return shipment (allowing room for batting.)
• Wrap your quilt in a plastic bag and enclose my address inside the plastic bag.
• Schedule your shipping to ensure your quilt arrives before your scheduled date. This will enable me to start your quilt promptly.
• Your finished quilt will be shipped by United Parcel Service (UPS) and insured for $200 per quilt. Shipping, handling, and insurance charges will be provided on your invoice.
